Spa treatment after knee replacement

After a total knee replacement or another knee operation, the joint needs one thing above all: regular, guided training of flexion and the thigh muscles. Marienbad’s spa cure bundles daily individual physiotherapy with exercise therapy in warm water, where buoyancy relieves the freshly operated knee.

How the cure works

Initial examination on arrival day

The spa physician reviews the surgical report, the angle of flexion and the surgeon’s clearance for weight-bearing, then puts together the treatment plan. Bring your hospital discharge letter; without it the plan is more cautious.

Movement in warm water and on land in alternation

Exercise therapy in the pool, where buoyancy relieves the knee while practising flexion, supplemented by lymphatic drainage against swelling and electrotherapy for the quadriceps. A full bath such as the carbon dioxide bath is only an option once the wound allows it.

Gait training and strengthening the quadriceps

Individual physiotherapy with weaning off walking aids, stair training and targeted muscle building around the knee joint. Load increases step by step, following the surgeon’s clearance plan.

Weekly check-up and final report

Once a week the physician checks the angle of flexion and strength and adjusts the plan. At the end you receive a report for your orthopaedic surgeon and a home exercise programme.

Treatments used

Individual physiotherapy
Daily, with targeted flexion training and building of the quadriceps, which loses strength quickly after knee surgery.
Exercise therapy in water
Buoyancy takes load off the freshly operated knee, so flexion can be practised that would still hurt on dry land.
Gait training
Correcting the gait pattern, stair training and gradually weaning off walking sticks.
Lymphatic drainage
Manual treatment against the swelling that can be especially pronounced after knee operations.
Electrotherapy
Stimulation current to reactivate the quadriceps, which can be inhibited by the operation.
Underwater massage
Pressure-jet massage in a warm pool for the muscles around the operated joint.

How long, and when

For knee replacements (position VII/10 of the Czech indication list), Czech public health insurance covers 28 days of comprehensive spa care. For other conditions after knee operations, such as meniscus procedures, where outpatient or inpatient rehabilitation was not sufficient (position VII/11), it is likewise 28 days, with 28 days on repeat or 21 or 14 days with contributory care. Self-paying guests coordinate the length with the spa physician and the surgeon; the professional minimum for balneotherapy is at least 10 treatments over at least 10 days.

Experience and research under way

Aftercare following a knee replacement has been part of the core programme in Marienbad for decades; the spa physicians prescribe flexion training, gait school and exercises in water on the basis of the operation report and their clinical experience with this course. How the individual guest fares is not left unchecked: every treatment stay begins with an initial and ends with a final medical examination and a written final report, so the course is documented and the spa physician can adjust the plan along the way. Systematic evidence for spa medicine as a field is being built for the first time in a long while. The Institute of Spa Medicine and Balneology (Institut lázeňství a balneologie, v.v.i.), founded by the Karlovy Vary Region in 2019, has been running the first clinical study of this format in thirty years since 2026, together with Léčebné lázně Mariánské Lázně. It covers urological and nephrological diagnoses rather than the one described on this page: more than a hundred patients are examined before and after the spa stay for health status and quality of life, with prim. MUDr. Ladislav Špišák, CSc. as professional guarantor, and results are not yet available. The study runs within the Spa Research Centre project (2024 to 2027), funded by the Just Transition Operational Programme and working with the Czech Technical University, the University of West Bohemia and the Czech Academy of Sciences. What such work can produce is shown by the same institute’s earlier collaboration with the Marienbad spa houses: their joint study of spa rehabilitation after COVID-19 received the European Spas Association Innovation Award in 2021 in the Medical Spa Scientific Research category.

When a spa cure is not an option

  • A fresh wound without completed healing, or signs of infection
  • Deep vein thrombosis of the leg less than three months ago
  • No clearance from the surgeon for full weight-bearing
  • Acute joint effusion or acute inflammation in the operated knee
  • Pregnancy, or an inability to manage personal care without a companion

What is the right spa cure after a knee operation?

For conditions after knee replacement or other knee operations, the Czech indication list provides separate positions (VII/10 and VII/11), and Marienbad treats them with daily individual physiotherapy, exercise therapy in water and gait training. Which programme fits depends on the procedure and your current angle of flexion, and is set at the initial examination.

How long does rehabilitation after a knee replacement take?

There is no standard length — it depends on how healing progresses, the type of prosthesis and your mobility when starting the cure. The Czech indication list provides for a 28-day comprehensive stay for conditions after joint replacement (position VII/10); how many treatments and exercise sessions you receive within it is set individually by the spa physician.

Can I go for a spa cure after meniscus surgery?

Yes, provided that outpatient or inpatient rehabilitation alone was not sufficient — that is the requirement for position VII/11 of the indication list, which covers conditions after meniscus and other knee operations. Your referring doctor submits the application; the spa physician decides on the specific treatment plan.

Does health insurance cover spa treatment after a knee operation?

For knee replacements it is 28 days of comprehensive spa care (position VII/10); for other knee operations without sufficient rehabilitation success it is also 28 days, or 28 on repeat, or 21 or 14 days with contributory care (position VII/11).
